I have been dealing with ed at http://www.exoticblanks.com/  (I can't post links or photos yet.)

Ed is in Racine wisconsin and uses flat rate shipping.  I have not had any errors with any of my orders.  He spent a good half hour talking to me one night about turning some very brittle materials like coral and fossilized bone.  Great guy, excellent service and some of the best prices I have found on pen kits.  Did I mention he sends candy with every order?
